They include long narratives by important characters, as well as create the idea that the events in the story actually happened or were a part of history. Epic heroes are considered as better than the average person and usually embark on a quest that changes them for the better. For example, the story of Gilgamesh is considered an epic because it is centered on the†¦show more content†¦The story of Polyphemos and Odysseus plays a significant role in The Odyssey because it influences the character of Odysseus, particularly the event of his escape. For example, the event causes Odysseus to become so prideful that he reveals his real name, an act that causes him problems later in the story. Polytropos: Polytropos, which translates to many forms, can also mean multiple personalities or can be used to describe someone who is readily adaptable to their situations and environment. An example of the importance of polytropos can be seen in the character of trickster gods. Considered as one of the male divines, trickster gods rely heavily on being able to have many forms and being adaptable. Romance: Similar to an epic, romance is a form of narrative that revolves around the idea of a hero embarking on a quest. In comparison to an epic, romance focuses on heroes who have been rejected by society and it’s conventions. In addition, rather than focus on the development of the hero such as epics do, romance narratives focus more on the adventures and quest themselves. Part II. Use of Inhaler With and Without Spacer-Free-Samples-Myassignment

Question: Disuss about the Use of Inhaler with and without spacer. Answer: The work conducted by Berger et al. has shown that the use of inhaler with or without spacer rarely has any significant effect on the patients. He had conducted an experiment to study the bronchodilator effects by the use of MF/F 100/10g with the use of spacer or without spacer. In comparison to placebo, it was found that indeed the medication had better broncho-dialatory effects in the participants in comparison to placebo but the presence of spacer did not make a significant impact. It was seen that when MF/F was used with spacer, it resulted in larger change in mean FEV1AUC012hr versus placebo that provided a mean difference of about 124 ml. Again when MF/F was used without spacer, it also brought successful effects over placebo and there was ultimately a main difference of 102 mL in mean adjusted FEV1AUC012hr . Therefore, the author had suggested that it did not bring out any difference with or without the use of spacer as the difference that had been noticed are not statisticall y significant. The difference is so small that it cannot be suggested to make a difference if used. However, the author had not denied any works which have showed that spacers are indeed more helpful than without the use of spacer with inhalers. In such explanations, he has provided data where he believed that modern spacers bring out more effects the non conventional spacers. However in his work, he had got the results that broncho-dialation was larger in patients with the addition of spacers than without the use of spacer but without any significant effect. He had suggested that proper clinical teaching of the patients to correctly use inhalers also has positive effects reducing the chances of use of inhalers. The authors had conducted an extensive study where they used the pressurized metered dose inhalers called pMDIs with the presence of spacers. It was earlier beloved that these combined methods are very much helpful for coordinating inhalation with actuation. The method that they selected here mainly consisted of using of the spacer with that of either extra-fine or fine articles inhaled corticosteroids called ICSs. Experiments were conducted by the use of extra-fine particles ICS with and without spacer in about 2090 participants. The next set of experiments used the 2 arms of fine particles ICS cohort study using 444 participants. In the previous case of the use of extra-fine particles no differences in rate was observed between spacer as well as non spacer arms and similar such occurrence also took in case of the fine particles ICS. These made them to conclude that no evidence is found which would support the fact that the use of spacers improved asthma outcomes for both the extra- fine as well as fine particles of the ICS administered by the pMDI. These had helps the researchers to stand against the long standing assumptions that spacers are able to improve the pMDI effectiveness. They also reject the thinking that there is also a need of pragmatic trials of spacers in clinical practices. Work or Levi et al. (2013) had a different take on the entire scenario for the experiment that he conducted had separate set of results which were not in lieu of the findings of the above two articles proposed by famous researchers. The scientists have actually selected participants where certain portion was taking pressurized metered dose inhalers after a good amount of training. Separations of the participants were done where they used pMDIS alone and the other groups used breath actuated pMDIs with the use of spacers. They had analyzed the results which have shown a striking result different for the other two. It showed that a greater proportion of the patients who have utilized the spacers along with their pMDIS have much better control over their conditions in comparison to those who have used the pMDIs alone. Another important fact that came out, which although not important in the assignment, is that breath actuated pMDS brings better result than conventional pMDIs. They have found that although patients who used breath-actuated pMDIs and spacers are low, a statistically significant difference in the numbers attaining asthma control is noted that proves the efficiency of the intervention. Therefore, researchers are of the opinion that those patients, who are not being benefitted from the training of using pMDIs alone, should definitely use spacers with them to increase the efficiency. Rahmati, H., Ansarfard, F., Ghodsbin, F., Ghayumi, M. A., Sayadi, M. (2014). The effect of training inhalation technique with or without spacer on maximum expiratory flow rate and inhaler usage skills in asthmatic patients: a randomized controlled trial.International journal of community based nursing and midwifery,2(4), 211. Rahamati et al., have stated in the year 2014 that the utilization of proper inhaling technique and proper knowledge of the patients about the rise of the inhalers are far much important that the controversy of the use of Metered Dose Inhaler (MDI) with or without the use of spacers. He mainly conducted a very simple research where he took patients suffering from asthmatic attacks often and grouped them into three. The first group was the control group, the second groups had people where training was given to handle only MDIs and the other the third group were the patients whom training was given to use MDIs and spacer together. After assessing their knowledge and functional skills in handling the techniques if inhalation of medication, they were given three training sessions. After that, their ability to use MDI was assessed. A quantitative data analysis showed striking results which still carried in the confusion of the efficiency if the use of spacers. It showed that the process t o take education through inhalers developed to a large extent due to the training and therefore the results shown by the second and third group was much higher than the control groups who showed poor efficiency rate in the skilled taking of the medication. No difference between the second and third group in the health of the patient was noticed. The intervention providing skills developed in the two groups but both the methods showed similar results which means that the combined provision of MDI with spacer did not bring any additional benefit than the use of MDI alone when both the groups become skilled enough to use the inhalers properly. This study conducted by Crane et al. (2014) has revealed that the participants who were mainly aged and over 55 years old, it was helpful for them to use spacer along with that pMDIs. This is because the results from the study have shown that patients who only took education in using pMDIs did not do better than those who used the device along with the spacer. The study also showed that dry powdered devices brought the best results after training programs. However, in the context of the discussion, the researcher had clearly stated that spacer use along with pMDIs bring out more additional benefits and are therefore more advantageous to the older citizens. Reflection: The use of PICO framework is immensely helpful in finding the right types of articles which will provide us with the best and authentic information. This framework basically acts as a filtering media which helped me to correctly choose the articles which were proper covering the criteria that I wanted to include in the bibliographies. I also got a large amount of help from the framework as it helps me to clearly understand and note down what my main motive was to incorporate in the study and helped me to develop a mind map about how to include all the specific details that were needed to support the scenario provided to us. It helped me to set p a comparative mode of discussion where the articles were searched entirely in a way which matched with the search question set by the Pico. In short, it narrowed my search, clearly depicted what I needed to check out for and how I could find the correct articles with the help of the question set. References: Levy, M. Wellness Industry in India Essay Example Paper Wellness Industry in India Essay Introduction INTRODUCTION In India, a country where traditional medicinal and health practices like Ayurveda and yoga have promoted the idea of mental and physical wellbeing since ancient times, a new concept of wellness is emerging. No longer limited to health, nutrition and relaxation, the new multi-dimensional definition of wellness encompasses the individual’s desire for social acceptance, exclusivity and collective welfare. WELLNESS INDUSTRY The 2009 FICCI-Ernst Young (EY) Wellness – Exploring the Untapped Potential report classifies the Indian wellness industry into two segments: wellness services and wellness products. Expected to grow dramatically in the next years, both the segments offer great opportunities to wellness providers. At the end of 2008, the overall industry was estimated at around INR 27. 000 crore (EUR 4. 05 billion1), of which INR 11. 000 crore (EUR 1. 65 billion) represented by the services segment and the rest by the products segment. The industry’s disorganization and fragmentation open further opportunities for international wellness players to capture a large share of the market. Wellness Industry in India Essay Body Paragraphs Wellness services From massages to cardio sessions, from steam baths to ago puncture, from slimming programs to beauty treatments, the wellness services segment includes all the facilities, centers and in general domestic and international players which offer Indian customers wellness solutions. Recent trends in the wellness services sector Spas: As per the FICCI-EY study, rejuvenation services – including spas, alternative therapies, Ayurveda treatments and beauty services – is expected to witness a 30 percent growth till 2014. In its 2009 report, SpaFinder Inc. counted over 2. 300 spas operating in India, with over 700 to open by 2012 and generating revenues for approximately EUR 264 million annually. No longer exclusivity of 5-stars hotel guests, standalone spas with a more affordable price-value equation are filling a gap that was missing in the market2. Gyms: Assessed at around INR 500 crore (EUR 75 million) in 2009, the gym market in India is quickly developing. As per the FICCI-EY study, fitness services – comprising gyms and slimming centers – will grow by more than 25 percent till 2014. According to the consultancy firm Deloitte and the US-based International Health, Racquet Sportsclub Association, the number of people using health clubs in India currently stands at 0. 23 million. Of this, experts say 40 percent are women3. HEALTH CLUB USERS PENETRATION RATE 2 3 Wellness industry unaffected by recession, liveMint, The Wall Street Journal, Jun 14, 2009. Hotels and restaurant: A further trend, emerging as a consequence of the significant number of lifestyle diseases which affects the country — India accounts for 60% of global cardiac illnesses and has over 50 million diabetics — is the emergence of hotels as health destinations, offering special menus and fitness programs4. Many restaurants are also starting to propose their health-conscious customers special menus. Main international and national players of the wellness services sector Overall, more than 15 international players in the wellness product and service space have entered India in the recent past and are aggressively expanding in the country. Just to name one, the US-based largest international gym chain in the world, Gold’s Gym has opened 48 gyms since its arrival in India in 2002. This increasing competition from multinationals has compelled national players – including Reliance, Dabur, Manipal Group of Companies, Dr. Batra’s, Kaya Health Clinic – to strengthen and speed up their market expansion strategies. To support the growth, after Talwalkars– one of the leading gym chains in India, with over 100 branches across 50 cities in the country, and over 100. 000 members – and Birla Pacific Spa, also the healthcare and beauty treatment provider Goodwill Hospitals, the drug retail chain Apollo Pharmacy, the diagnostics chain Super Religare Laboratories and the eye care firm VasanEyecare are planning to go public in the next 12 months5. 4 Wellness on the menu, liveMint, The Wall Street Journal, Jul 29, 2011. 5After Birla Pacific Spa and Talwalkars niche beauty and wellness companies planning for IPO, The Economic Times, Aug 25, 2011. While sub standard facilities and a lack of qualified staff still characterize the domestic wellness scene, this unsatisfying picture opens further opportunities to international players, which will bring to India their sets of standards of procedures and training programs. Wellness products Including various types of â€Å"modified† foods (e. g. , fat free, low calories, energetic, reinforcing), oils, supplements, integrators, and personal care products, wellness products are gaining more and more space in Indians’ purchase basket. No longer limited to preventive or supportive nutrition, the wellness product portfolio reflects a mix of indulgence, invigoration and narcissism6. Sugar-free products With diabetes increasing at an alarming rate –the number of people with diabetes was over 45 million in 2010 and is expected to rise to 69. 9 m illion by 2015 –, obesity, hypertension and other lifestyle-related disorders, sugar-free product variants are gaining more space on the retail shelves. While Indians have begun to show a liking to sugar-free products, sugar substitutes (often referred to as â€Å"sweeteners†) do not receive the same welcome. In fact, sweeteners are not yet considered a low-calorie sugar substitute, but a product for people suffering of lifestyle diseases8. Nutraceuticals market Within the wellness products segment, the nutraceuticals market – merger of the words â€Å"nutrition† and â€Å"pharmaceutical† indicating functional foods, beverages and dietary supplements which provide health and medical benefits – is expected to grow exponentially. The 2009 FICCI-EY Nutraceuticals – Critical supplement for building a healthy India estimated the market to be around INR 44 billion (representing one 7 8 Insights on Indian market, NutriConnect, 2009. Moreover, due to their high prices compared to conventional foods, the lack of credibility of their benefits among costumers, and the not-yet-implemented regulatory framework, the latent market for nutraceutical products is estimated to be two to four times the existing market size. CONCLUSIONS In India, a new multi-dimensional definition of wellness is emerging, encompassing the individual’s desire for social acceptance, exclusivity and collective welfare. Be it in the services or product sector, the wellness industry in India is showing bright opportunities to wellness providers, especially international players. 9 WHO statistical information system, 2008. Maintaining a Tree Through the Next Decade

Maintaining a Tree Through the Next Decade Specimen trees in the landscape need consistent care over time to ensure their continued health, proper conditions for growth and to prevent hazardous conditions that threaten surrounding property. Here is a tree care schedule developed by the United States Forest Service for a tree owners use and listed according to tree care type. Watering the Tree The key to newly planted tree survival is providing adequate water. Although the first 3 years are most critical, a trees watering needs should be maintained for life. Initially, a newly planted tree needs to be properly watered with an adequate amount to pack the soil, to remove root-drying air and to moisten the root ball. On adequately draining soils, 5 gallons of initial water should be enough. Fast draining soils may need more frequent watering than a slow draining soil. Year 1 - 3: It is most critical to provide adequate water during the annual growing season, between late spring and autumn.Year 4 and after: You can relax a bit on tree watering in later years but water may be needed during long periods of drought. Mulching the Tree Mulching a newly planted tree ensures that moisture is available to roots over time and reduces grass competition. A good mulch (organic materials like leaves, bark, needles and fine wood chips) should ring the tree base (over the critical root zone) but never touch the tree. No fertilizer is necessary when quality composted mulch is used. Year 1 - 3: Maintain the mulch level with no more than 4 inches of material over the roots (the wider the better) but not touching the tree.Year 4 and after: A tree appreciates a good mulch so it is appropriate to maintain adequate mulch levels annually during spring. Avoid using nitrogen fertilizers - use complete fertilizers only after a soil test. Staking the Tree Not all newly planted trees need staking to remain standing straight. Stake only if the root ball is unstable or the tree trunk is bending. Use only loosely tied, wide straps and limit the number of straps to a minimum for support. Year 1 - 3: Use tree stakes only when needed. Many tree owners automatically stake every tree not knowing that it is often unnecessary. Check all stakes and straps during spring and autumn for loose fit and alter to prevent trunk damage. All straps should be removed after the first or second year.Year 4 and after: Do not stake older trees. Cleaning the Root Collar Roots that encircle the trunk at the root collar can cause tree health and safety problems. A trees root collar is its transition zone between stem and root at the ground line. Proper planting depth can go a long way toward keeping the root collar clean and free of encircling roots. Remember that piling soil or mulch against the root collar encourages strangler roots. Year 1 - 3: Proper planting and mulching will go a long way toward solving most root collar problems. The first several years of growth after planting is when tree collar problems develop, so keep the collar exposed by removing soil and mulch. Overfertilization can speed up the process and make the condition worse.Year 4 and after: Revisit and check the root collar every 4 years. Use a hand trowel to loosen and remove the soil around the base of the tree until the first set of roots is uncovered. Inspecting Tree Health Checking a trees health may not only be subjective for a novice but determining the health of a tree is complicated and should be done by an expert. Still, there are things you can do that will alert you to tree health problems. Ask yourself these questions when inspecting a tree: Is the current years growth much less than in past years growth? Although fast growth does not necessarily mean good health, a dramatic reduction in growth rate may be an indication of poor health. Are there dead limbs, odd colors on leaves and bark or a patchy crown? These tree symptoms can be the first indicators that a tree is unhealthy and should be inspected in detail. Remember that planting a healthy tree from the beginning is the best way to assure its future health. Pruning the Tree When pruning a newly planted tree, only prune critical branches and no others! Critical branches are those that are either dead or broken. You can also remove multiple leaders to leave only one central stem. It may be best to postpone pruning to avoid transplanting shock due to loss of leaves. Year 1 - 3: Prune only critical branches or to eliminate extra leaders in the trees first year. You will have plenty of time to form your tree so only prune lightly in Year 2 or 3.Year 4 and after: Prune your tree for form and function every three years. As a rule of thumb, prune fruit trees every 1-3 years, prune deciduous shade trees every 5 years and evergreens only as needed.
